Client Rights & Compliance Information

At Lopez Wellness, PLLC, we are committed to providing ethical, professional, and compassionate care. Most concerns that arise during therapy can often be addressed directly through open communication. If you have questions or concerns about your care, we encourage you to discuss them with your clinician so we can work toward resolution together.

If you feel that your concerns have not been resolved, you have the right to file a complaint with the appropriate agencies. The following information is provided to support you in that process.

Business Practice Concerns

If you believe you have been a victim of fraudulent or unethical business practices in the State of Texas, you may file a consumer complaint with the Texas Attorney General’s Office.

License or Ethical Concerns

If you believe a clinician has violated ethical standards or acted outside the scope of their professional license, you may file a complaint with the appropriate licensing board.

Requesting Medical Records

Lopez Wellness, PLLC maintains client records in accordance with Texas state law. Records are retained for:

  • 7 years for adult clients

  • At least 5 years after a minor client turns 18
